An Orthopaedic Surgeon, also known as an Orthopaedic Consultant or a Consultant Orthopaedic Surgeon, is a bones and joints specialist. At The Sarum Road Hospital, our Orthopaedic Consultants specialise in certain joints. So, if you visit us for help with hip pain, you will be treated by a Consultant with a special interest in hips.  

At your initial consultation, your Surgeon will ask about your general health and look at your medical history to see whether you have any existing medical conditions, as well as if you take any treatment for hip pain (and whether this has helped you manage your pain). They will want to know more about the nature of your pain, including when it started and how it impacts your daily life.   

In order to assess your symptoms and make an accurate diagnosis of your hip pain, your Consultant will carefully carry out a physical examination of your joint(s), and request an X-ray, if needed. Your X-ray will be performed by one of our Radiographers.   

Although the prospect of your initial consultation may feel daunting, it doesn’t have to be this way. This is a safe opportunity for you to ask your Consultant any questions you might have about your hip pain. They will share detailed information about your condition and potential treatment options. After your Consultant has determined the cause of your hip pain, they will explain more about hip replacement surgery and whether it might be the right treatment option for you.

Before hip replacement surgery, you will either be administered general anaesthetic (which means you’ll be asleep for the whole procedure) or spinal anaesthetic (which will numb your lower body). If you are given spinal anaesthetic, you will also be given a sedative.  

There are two types of hip replacement surgery: total and partial. The type of surgery you have will depend on the extent of damage to your hip joint and which parts of it are affected.  

As your Consultant will explain to you, a hip joint is a ‘ball and socket’ joint, where two bones meet: your femoral head (ball) and your acetabulum (socket). 

Partial hip replacement (half hip replacement) surgery


During this form of surgery, your Consultant Orthopaedic Surgeon will make an incision on the side of your hip and remove your damaged femoral head, which will then be replaced with an artificial joint, known as a prosthesis. This prosthesis is usually made of plastic, metal or ceramic, or a combination of these materials.  

The different components of a hip prosthesis can be cemented or uncemented. Cemented components are fixed to your healthy bone using special “cement.” Uncemented components have a rough surface that allow your bone to grow onto it, holding it in place.  

Total hip replacement surgery


In the case of a total hip replacement, your Consultant will remove both your femoral head and your acetabulum and replace each with their respective artificial component.   

Whether you undergo partial or total hip replacement surgery, at the end of the procedure your Consultant will bend and move your leg, making sure it moves properly now the prosthesis has been implanted. They will then close their incision with dissolvable stitches. The outermost layer of your skin will be closed with glue or sutures.   

How long does a hip replacement take?


A hip replacement operation typically lasts for one to two hours.   

Every person's hip replacement recovery timeline looks different depending on their individual health and circumstances.   

Your Orthopaedic Surgeon will have taken the time to fully understand your personal circumstances and so they will be able to tell you what your hip surgery recovery timeline will look like.  

At The Sarum Road Hospital, you will be supported by a multidisciplinary team of healthcare professionals throughout your recovery journey. This includes aftercare at home, where you will be advised to avoid certain strenuous activities and to perform specialist exercises set by your Physiotherapist.   

Most people can leave hospital and go home two to three days after having hip replacement surgery. Within two weeks, you will be able to walk more regularly, but you will require the use of a walking aid, such as crutches or a walker. Please be gentle with yourself when walking. You can reap the benefits of exercise without walking fast: very low impact exercise is key at this stage in your recovery.  

After two to six weeks, you can begin incorporating gentle exercise into your daily routine, such as long walks or swimming. You can also usually return to work at this stage, depending on the type of work you do (if you have an office job you can return to the office, but for a manual job you might need more time off).   

After six to 12 weeks, you should be able to carry out everyday activities with ease.

Your Physiotherapist will work with you to build a tailored exercise plan following surgery. These will help strengthen your hip muscles and improve your mobility. Your Physiotherapist will also show you how to incorporate exercise into your daily routine at home, using specialist equipment if required.   

They will advise on how regularly you should perform these exercises outside of your sessions, as well as any equipment you might need.
