What is Mohs surgery?

Mohs surgery, also known as Mohs micrographic surgery (MMS), is a highly precise technique used to treat certain types of skin cancer, including:

  • Basal cell carcinoma (BCC)
  • Squamous cell carcinoma (SCC)
  • Recurring, complex, or rare skin cancers
  • Cancers in areas where it's important to remove as little tissue as possible, for example, near the eyes, lips, ears, or nose

The procedure involves removing cancerous tissue layer by layer and examining it under a microscope until all cancer cells are removed, ensuring maximum preservation of healthy tissue and enhancing outcomes, including scarring.

The surgery has been recognised as the skin cancer treatment with the highest reported cure rate for certain tumours, including basal cell carcinoma. However, it is important to remember that no cancer surgery has a 100% success rate in battling the disease.

The total cost of Mohs surgery treatment will depend on your individual circumstances. At the time of your appointment your consultant will discuss all options with you and ensure all your questions are answered.

Our fixed-price packages include the cost of your surgery and all appropriate aftercare appointments. However, any pre-surgery diagnostic tests and your consultant's outpatient appointment consultation fee are charged separately.

What to expect during your initial consultation

During your initial consultation, you will meet a specialist consultant to discuss your symptoms, medical and family history, as well as your lifestyle and habits that may have put you at risk of skin cancer to help guide your individually tailored treatment plan.

Your consultant will assess this information and perform a physical examination to check for changes in the skin so they can confirm a diagnosis and determine if Mohs surgery is a suitable option for you.

How to prepare for Mohs surgery

Before having surgery, it is important to tell your consultant about any medication you are currently taking. This is essential, especially if you are taking antiplatelet medicines, such as aspirin or clopidogrel, or anticoagulant medicines, such as warfarin or rivaroxaban. These medicines can increase the risk of bleeding during and after the procedure.

If surgery is being done on the face, we advise you don’t wear makeup or apply skincare before the operation. If you wear glasses or contact lenses, please bring these with you as the dressing may prevent you wearing your glasses initially.

You will need to arrange for someone to drive you or take a taxi home. It is advised you do not take public transport or drive yourself home after surgery.

What happens during Mohs surgery

Before beginning the procedure, local anaesthetic will be used to numb the area. This means you will be awake during the procedure, but your surgeon will make sure you are comfortable and reassured throughout the surgery.

If your tumour is near the eye, we might also use anaesthetic eye drops.

Once the area is fully numb, your surgeon will remove the cancerous tissue in stages. This includes taking away the visible portion of the cancer as well as a thin layer of tissue underneath and around the cancer.

The tissue is then taken to our onsite laboratory, where each layer is checked under a microscope for cancer. If results show that there are cancer cells in the surrounding tissue, we’ll continue with the surgery to remove another layer of tissue and send it to the lab.

This process is repeated until the results show there are no more cancer cells.

Once all cancer is removed, we can perform skin reconstruction of the affected area on the same day as your procedure. Skin reconstruction helps to restore function and improve the area’s appearance after surgery.

Recovering from Mohs surgery

After the surgery, you may experience some mild pain and soreness in the area where the tissue was removed. You can take over the counter pain medication, such as ibuprofen or paracetamol, to help manage the pain.

Severe pain after surgery is not common. We will provide relevant contact details in case you experience severe pain or any other side effects.

Your surgical wound may be dressed for about a week or longer. The exact length of time you'll need a dressing will depend on your individual recovery. Your surgeon will give you specific instructions on how to take care of the wound following the procedure.

It’s important to rest after having Mohs surgery. It is advised to take at least 2 days off work until you’re fully recovered. Your consultant will be able to advise you on exact timescales, depending on your own individual circumstances and recovery journey.
