Circle Health Group Patient Terms and Conditions
1.Who do these Terms apply to?
1.1 These Terms apply to you if you are:
(a) a self-pay patient receiving Services as part of a Fixed Price Package;
(b) a self-pay patient receiving Services other than as part of a Fixed Price Package;
(c) an insured patient;
(d) an international patient; and/or
(e) an NHS patient;
if you fall within (a) to (d) above, you will be deemed to be a "Private Patient" for the purposes of these Terms.
1.2 While it is important that you read these Terms in full, please pay particular attention to the following sections:
(a) Section 2: Consultants
(b) Sections 3.3 – 3.7: Is the Fixed Price liable to change?
(c) Section 5.3: Cancellation Rights
(d) Section 9.7: Cancellation Fees
1.3 These Terms reference the Hospital's standard rates. Further information on the Hospital's standard rates can be found by contacting the Patient Liaison Officer at your chosen Hospital.
2. Consultants
This section is relevant to you if you are a Private Patient.
2.1 While at one of our Hospitals, you will be under the care of the Consultant(s) you have been referred to. Your Consultant(s) may also involve other consultant(s) in your care, if appropriate. Circle Health Group staff, including nurses, will provide your care under your Consultant’s instructions.
2.2 Consultants involved in your care, who may be physicians, surgeons, radiologists, general practitioners or anaesthetists (we refer to these medical practitioners as “Consultants” in these Terms), are independent medical practitioners and they are not employed by Circle Health Group. Your Consultant(s) offer their services to you direct as a private clinician and they contract with you separately in respect of their services. As such, they are responsible for the care and treatment that they prescribe, recommend or provide to you. In particular, if your care and treatment includes surgical intervention, then your Consultant will be responsible for the surgical aspects of your care and treatment. If an anaesthetist is required, that Consultant anaesthetist will be responsible for the anaesthetic care and treatment and will contract with you separately for their services.
2.3 Where we talk about, or refer to, the responsibility of your Consultant in these Terms, this means that your Consultant will be responsible (i) under the terms of their contract with you (which may or may not be in writing) and /or (ii) for any negligence or wrongful act or omission in the care and treatment they provide to you. Circle Health Group will not be liable for any act or omission of Consultants (or the company, partnership or other entity that employs or engages your Consultant(s)) in contract or otherwise.
2.4 Unless we advise you otherwise, your Consultant(s) will charge you separately for their services in connection with your care and treatment, including for the initial consultation. Any diagnostic tests or investigations conducted as part of, or related to, your initial consultation will be charged to you separately by the Hospital.
2.5 Our invoice under the Fixed Price Package will include your Consultant(s') fees while you are in the Hospital. In this event, and in any other instances where our invoice includes your Consultant(s') fees, we are acting as your Consultant(s') agent only in collecting those fees. Your Consultant(s) will at all times remain an independent medical practitioner.
2.6 Your Consultant(s) and their secretarial staff do not have authority from us or the Hospital to quote for hospital charges. Any hospital prices mentioned by them are subject to written confirmation by the Hospital.
3. Self-Pay: Fixed Price Package
This section is relevant to you if you are a self-pay patient and we have agreed in your Admission Letter or your Outpatient Communication that we will provide the Services to you for a Fixed Price.
Fixed Price and Payments
3.1 We will supply the Services to you as set out in your Admission Letter or your Outpatient Communication (as applicable) which will detail the services that you will receive from us under the Fixed Price Package.
3.2 Your Admission Letter or your Outpatient Communication (as applicable) will also set out (amongst other information):
(a) the Fixed Price that we have calculated for your Fixed Price Package;
(b) appointment or admission dates; and
(c) methods of payment.
Is the Fixed Price liable to change?
Admitted patients
3.3 Following receipt of your Admission Letter and prior to your admission date, you will have a pre-assessment. In the pre-assessment you will be asked questions about your health and medical history and you will be provided with further information on your procedure and/or admission.
3.4 Following the pre-assessment, we may consider (for clinical or medical reasons) that it is not appropriate to continue with the Services or it is not possible to provide the Services at the Fixed Price stated in your Admission Letter. We might also consider that further tests and/or assessments are required. Where this is the case, we will discuss this with you.
3.5 If we consider that we are no longer able to provide the Services, we will tell you the reason(s) for this decision.
3.6 If we are still able to provide the Services to you, but at a later date (for example, following further tests and/or assessments) and/or at a different price to that stated in your Admission Letter, we will provide you with a new Admission Letter with updated information. Where we are no longer able to offer the Services for a Fixed Price, we will provide you with costs information in accordance with Section 4 below.
3.7 Your Fixed Price Package and the Fixed Price are subject to the results of your pre-assessment (which is explained in more detail in Sections 3.3 and 3.4 above). After the results of your pre-assessment, your Fixed Price will not change. We calculate the Fixed Price based on:
(a) your Consultant(s)’ medical advice;
(b) required post-operative care;
(c) the results of any scans and diagnostic tests; and
(d) the complexity of your care and treatment.
What is Included in the Fixed Price?
3.8 The Fixed Price for the Fixed Price Package includes:
(a) the following Services that we provide to you in accordance with our contract with you:
(i) any pre-assessment(s) provided by us to determine your fitness for the procedure immediately prior to your admission, if necessary;
(ii) your accommodation and meals at the Hospital for as long as you are required to stay at the Hospital (as advised by your Consultant(s));
(iii) your nursing care while you are in the Hospital;
(iv) the Hospital’s theatre facilities, drugs and dressings while you are in the Hospital;
(v) any required prosthesis (approved by us) typically used for the procedure that you are having;
(vi) imaging, such as x-rays or scans, physiotherapy, pathology and histology needed while you have been admitted to the Hospital;
(vii) basic walking aids where clinically required (such as walking sticks or crutches and wheelchairs for use in the Hospital) but not home aids or larger items (such as stair lifts);
(viii) take-home drugs advised by your Consultant(s) for up to fourteen days after discharge and antibiotics for the stated period prescribed by your Consultant; and
(ix) any follow up tests and scans that relate to your care, treatment or procedure, where clinically required.
(b) Services that your Consultant(s) provides to you under your separate contract with them but that are included within the Fixed Price:
(i) any care or treatment (other than high dependency, intensive or critical care) carried out in other Circle Health Group locations, provided it is part of the care and treatment plan devised by your Consultant(s) for you as part of your Fixed Price Package at the Hospital;
(ii) any care required in the Hospital’s high dependency unit or intensive care unit;
(iii) your Consultant(s)’ or other healthcare professional’s operating fee and anaesthetist’s fee while you are in the Hospital;
(iv) up to six months post-operative care where clinically required by your Consultant(s) and/or other Circle Health Group healthcare professional involved in your care provided it takes place at the Hospital or another Circle Health Group location, including:
(A) removal of stitches, dressings or plaster, if required;
(B) one follow-up consultation with your Consultant, where clinically required; and
(C) treatment for any clinical complications, as we explain in Section 5, below.
What additional costs might I be responsible for that are excluded from the Fixed Price?
3.9 The Fixed Price for your Fixed Price Package excludes:
(a) diagnostic tests or services received prior to your admission (and prior to and not forming part of your pre-assessment). These will be invoiced separately directly by us for the Hospital’s fees and by your Consultant(s) (or by us as agent for your Consultant(s)) for the Consultant(s)’ fees;
(b) your Consultant(s) or any other healthcare professional’s fee for the initial outpatient consultation;
(c) any care or treatment provided anywhere other than at the Hospital or other Circle Health Group location (unless any part of your care was electively planned at an NHS hospital as part of your procedure) or any other care or treatment at any other hospital (including the NHS);
(d) any treatment or care or long-term care that is not related to your Fixed Price Package;
(e) any drugs or medication not included within the take-home drugs as described in Section 3.8 above;
(f) convalescence, treatment, accommodation or meals provided after your Consultant(s) has advised that you are fit for discharge;
(g) visitors’ meals, and other sundries;
(h) ambulance fees;
(i) any replacement prosthesis or other items where required due to normal wear and tear; and
(j) any costs or fees not specified as included in Section 3.8 above or in your Admission Letter or your Outpatient Communication (as applicable), and any provision of any of the above by the Hospital will be charged separately to you at the Hospital’s standard rates and you will be responsible for payment of those charges.
3.10 Any medical treatment not related or connected with the care and treatment identified at your pre-assessment or Admission Letter or your Outpatient Communication (as applicable) which requires a separate pathway of care, or is not clinically required, whether or not carried out at the Hospital, even if you are not discharged from the Hospital, is not included in the Fixed Price Package.
3.11 Costs incurred for services and items provided to you by the Hospital that are not included in the Fixed Price will be charged to you at the Hospital’s standard rates.
4. Self-Pay: Other payment arrangements
This section is relevant to you if you are paying for the Services (other than when they are covered by a Fixed Price Package or paid for by an insurer directly).
What pricing information will be provided to you?
4.1 If the Services are not paid for by an insurer directly, are not covered by a Fixed Price Package or are otherwise to be paid by you in accordance with these Terms, you will be charged at the Hospital’s standard rates. Your Admission Letter (if relevant) or your Outpatient Communication will confirm this.
4.2 Unless otherwise indicated, your Consultant(s) will invoice you separately for the services that they provide.
4.3 We will give you an estimate of costs for your Hospital care in your Admission Letter or your Outpatient Communication. Your Consultant(s) will invoice you separately for their services. Please note that it is not always possible to give an exact estimate of costs for the care you receive at a Hospital (or other Circle Health Group location) and the total cost may depend on a number of factors, including any other medical conditions you may have. We will always try to provide an accurate estimate and if the cost of your care is likely to exceed this estimate we will try to notify you as soon as possible. You are responsible for the payment of all care you receive at a Hospital (or other Circle Health Group location) and following discharge, including any sundry items.
When will you need to pay for the Services?
Outpatients
4.4 If you are an outpatient, you will need to pay, prior to or on the day you attend the Hospital, the fees specified in your Outpatient Communication.
Admitted Patients
4.5 If you are admitted to the Hospital (or other Circle Health Group location), you will need to pay the estimate of the costs of your care prior to admission.
4.6 If the costs you have paid do not cover the entirety of your anticipated stay in the Hospital (or Circle Health Group location), you may be asked to pay for your treatment in stages throughout your care and/or you will be required to settle your account on or within seven days of discharge from the Hospital (or other Circle Health Group location). You agree that we can debit any outstanding balance from your credit/debit card (which you are required to provide when you register at the Hospital (please see Section 9.6 below)) provided that we have given you seven days’ notice of our intention to do so.
4.7 If your Consultant(s)' fees are not included in your invoice (we have mentioned, in Section 2.5 the situations where our invoice will include your Consultant(s') fees), you will need to settle these directly with your Consultant(s).
5. All Self-Pay Patients
Complications
5.1 Any Fixed Price Package and other non-packaged price covers any medical or surgical complications directly related to your surgery for six months following your discharge from the Hospital (or other Circle Health Group location) following the procedure to the extent described in Section 9.2, provided that:
(a) as mentioned in Section 2 above, your Consultant is responsible (both under the terms of their contract with you and for any negligence or wrongful act or omission of the Consultant) for the care and treatment that they prescribe, recommend or provide to you;
(b) such post-operative care and complications are treated at the Hospital or other Circle Health Group location; and
(c) you have followed the advice of your Consultant and other Circle Health Group healthcare professionals involved in your care or treatment. The decision as to whether a complication is related to the care or treatment rests with your Consultant(s) or other relevant Circle Health Group healthcare professional(s) and staff.
5.2 If your care or treatment involves cosmetic surgery and this surgery does not match your Consultant’s expectations, revision surgery may be provided without additional charge (subject to approval of the Hospital’s Executive Director or Director of Clinical Services), provided this is identified by your Consultant to us within six months of your initial admission. If the cosmetic result of your surgery meets your Consultant’s expectation, any revision surgery will be chargeable at the Hospital’s standard rates and your Consultant(s) will confirm their charges separately.
What cancellation and/or refund rights do self-pay patients have?
5.3 If you decide not to go ahead with your care or treatment as part of your Fixed Price Package or as part of other, non-packaged self-pay services agreed with you, you may contact us at any time to cancel. You will be required to pay for any Services received up until the point of cancellation. This will be charged at the Hospital’s standard rates. We also reserve the right to charge you a cancellation fee. Details of our cancellation fees are set out in Section 9.7 and are charged to cover our costs arising out of the short notice cancellation.
5.4 We will refund any advance payment made by you or on your behalf, less any amount that you owe to Circle Health Group and/or your Consultant(s) where we have collected fees on their behalf. For Services that we have not yet provided to you we reserve the right to charge a cancellation fee as described in Section 9.7.
5.5 If your Consultant(s) cancels your treatment because they consider it is not in your best interests for medical reasons, and
(a) you have already paid for your Fixed Price Package, we will refund your payment. Note that you will not be required to pay for any Services included in your Fixed Price Package that you received up until the point of cancellation; or
(b) you have already paid for other non-packaged self-pay Services, we will refund your payment. Note that you will not be required to pay for any Services that you received up until the point of cancellation.
What cancellation rights do we have, and what refunds apply?
5.6 We will make every effort to provide the Services on the date that we have specified in your Outpatient Communication and/ or Admission Letter. However, we cannot promise this, and we reserve the right to refuse your admission and/or appointment or to cancel or change the date of your admission and/or appointment.
5.7 There may be delays or cancellations for any reason, such as an event outside our reasonable control, for operational or technical reasons or because your Consultant(s) does not think it is in your best interest for medical reasons. Where this happens or where we refuse your admission, we will try to give as much notice to you as possible. Where possible, as appropriate, we will always try to rearrange any appointment or admission dates with you. Where we are required to cancel the appointment or refuse admission, any advance payment you have made for Services that have not been provided will be refunded to you. We will refund these amounts by electronic transfer only to the cardholder or person who made the original payment.
5.8 Please note we do not issue any refunds in cash.
Flexible Payment Option
5.9 You may be eligible to spread the cost of your care or treatment using our flexible payment option with Chrysalis Finance. Details can be found on our website (https://www.circlehealthgroup.co.uk/paying-for-treatment/flexible-finance).
6. NHS treatment
6.1 If you choose to have your NHS treatment at one of our Hospitals in England, Scotland or Wales, all the costs will be covered by the NHS. You only pay for extra personal items, just as you would in any NHS hospital, and we will invoice you for these extra personal items.
Outside of the UK
6.2 If you are not a resident of the UK but you require any NHS treatment and it is not included in your Fixed Price Package, please note that you will be liable to pay any charges for any treatment or care carried out by the NHS, and we may invoice you for any costs that we incur as a result of any NHS treatment.
7. Insured Patients
7.1 You agree to pay for the Services.
7.2 Whilst you will remain responsible for the payment of the Services, where you have private medical insurance:
(a) we will, where possible, process the insurance claim for the Services with your insurer, provided you have given us and your insurer all the information we and your insurer need to do so, including but not limited to your policy and pre-authorisation numbers. If this information is incomplete or inaccurate, we may not be able to process your claim and you will need to pay for the Services, as set out in (c) below;
(b) where we process your insurance claim and your insurer pays us directly, the rate agreed between Circle Health Group and your insurer (rather than the Hospital’s standard rates) will apply to the Services; if your insurer fails to settle our invoices (or any part of them) within 45 days of the date of issue we will assume that the outstanding amount will not be paid by your insurer and we may invoice you directly or debit the relevant balance from your credit or debit card in accordance with the process set out in Section 9.6 below (as relevant); and
(c) if we invoice you for the Services or any element of them, you agree to pay us the amount invoiced within the time limits set out therein. If you do not think that we have invoiced you correctly, please let us know as soon as possible so we can investigate this further.
7.3 It is your responsibility to confirm with your insurer in advance that the Services are covered by your insurance policy; we cannot obtain any such confirmation on your behalf. While you are at one of our Hospitals, if you want to check with your insurer whether any aspect of the Services is covered by your insurance policy, we will give you access to a telephone so you can contact your insurer. You may be required to obtain confirmation of cover from your insurer for various aspects of the Services throughout your pathway. At this point, your insurer will inform you if you need to pay a shortfall for any aspect of the Services.
7.4 Circle Health Group will not be responsible for any insurer shortfall in cover. We recommend you confirm your policy limits with your insurer before you undertake your treatment.
7.5 Your insurer may require access to your medical records in order to validate and approve your treatment. Access also might be required to allow your insurer to audit Circle Health Group’s performance of the contract between Circle Health Group and your insurer. Any audit undertaken by your insurer will be for the purpose of validating the accuracy of Circle Health Group’s charges and assessing and assuring the quality of services provided by us or Consultants.
7.6 Please note that in some cases the care pathway determined by your Consultant(s), nursing staff and other medical professionals providing your care or treatment may not be covered by your insurance policy. This could mean that your insurer may not pay for certain parts of the care you receive, and you will be required to pay for that part of your care. In particular, you should note that treatment for complications may in some cases not be covered by your insurer and in such cases; you agree to cover the cost of the Services as set out in Section 7.2(c) above.
7.7 Please note that your insurance policy may not cover the cost of sundry items or other items such as specialist equipment, like crutches or wrist braces, or certain medications, or it may only cover part of such costs. You will be required to pay for any such items not reimbursed by your insurer.
7.8 If you pay for the Services and subsequently seek reimbursement from your insurer, and if no other rate has been expressly agreed between you and Circle Health Group or Circle Health Group and your insurer, the Hospital’s standard rates will apply to your care.
7.9 In some instances, Circle Health Group may provide discounts to and/or pay commissions to third parties under separate commercial arrangements. This does not affect your treatment or the amount that you pay for the Services.
8. International Patients
8.1 By signing a Registration Form and agreeing to these Terms you confirm that you have the necessary documentation to enter the UK and that you meet all relevant immigration criteria. You also confirm that you have made adequate arrangements to pay for the Services.
8.2 Circle Health Group may contact the Home Office or UK Border Agency (as relevant) to the extent necessary to clarify any information regarding your visa to enter or remain in the UK in connection with your care.
9. Other Terms and Conditions
Unless we expressly state otherwise, this Section applies to all patients
9.1 The Admission Letter will include confirmation of the date of your procedure. If you have not made a booking, you will also be sent a letter (and if you are a self-pay patient that letter will include our quotation) with details about how to book a date for your procedure, but you will always receive an Admission Letter.
9.2 While Circle Health Group and your Consultant(s) will always try to meet your expectations, we cannot guarantee the result of any procedure, care or treatment, and it is possible that complications with your treatment or surgery can occur. Your Consultant(s) will explain these to you before your treatment. Where you are a self-pay patient, we have set out the process for dealing with complications in Section 5. For insured patients, the process for dealing with complications in set out in Section 7.
9.3 The decision as to whether you are fit for discharge rests with your Consultant(s). If, with the agreement of the Hospital, you decide to stay at the Hospital beyond the date that your Consultant(s) considers is appropriate for you to be discharged, the Hospital’s standard charges will apply and you will be invoiced separately. If you discharge yourself against the advice of your Consultant(s) no further services will be provided as part of the Fixed Price Package (as appropriate), and no refund will be given if you leave the Hospital earlier than expected.
9.4 We will need certain information from you that is necessary for us to provide the Services. This can be asked for by anyone involved in your care or treatment, including by staff at the Hospital, by your Consultant(s) or other medical professional(s) or in your Outpatient Communication and/or Admission Letter. If you do not, after being asked by us, provide us with this information, or you provide us with incomplete or incorrect information, we may not be able to provide you with a full range of Services. As a result, we may be unable to see you at our Hospital. Of course, if you are a self-pay patient or an insured patient and we are unable to provide the Services to you, you (or, where applicable, your insurer) do not have to pay for the Services that we have not provided, but this does not affect your obligation to pay for any Services we have already provided. If you are a self-pay patient, any advance payment you have made for Services that have not been provided will be refunded to you.
9.5 We strongly advise that you avoid bringing any valuables or cash to the Hospital. If you do nonetheless bring any valuables or cash, this is at your risk as we do not accept any responsibility for the theft, loss of, or damage to, any of your or your visitors’ cash, valuables or any other property that you or your visitors bring to the Hospital.
9.6 If you are a Private Patient, we will ask you for your debit or credit card details when you register. Your details will be kept securely for up to six months or until any remaining balances are cleared and we will tell you if we intend to take a payment from your card before we do so.
Cancellation fees and additional payment terms
9.7 If you are a Private Patient, we reserve the right to charge a cancellation fee if you cancel any appointment with Circle Health Group (or an Affiliate) within seven days of your scheduled appointment or admission date. If we charge you a cancellation fee, the amount of the fee will be the greater of £250 or the cost of the care that you have received up to the point of cancellation and any other reasonable costs that the Hospital has incurred. This is to cover our costs arising out of the short notice cancellation.
9.8 We may cancel any appointment or admission date or any care or treatment at any time if you are a self-pay patient and you do not pay us when you are supposed to in accordance with your contract with us.
If there is a problem with the Services
9.9 If there is any problem with the Services we provide, please contact us and tell us as soon as reasonably possible, and we will investigate the problem under our complaints procedure and try to resolve the problem as soon as we can.
9.10 Please ask any member of staff at the Hospital for a leaflet about our complaints procedure. This can also be found on our website (https://www.circlehealthgroup.co.uk/contact-us/making-a-complaint).
9.11 You have legal rights in relation to Services not carried out with reasonable skill and care, or if the materials we use are faulty or not as described. Nothing in these Terms will affect these legal rights.
Events outside our reasonable control
9.12 We will not be liable or responsible for any failure to perform, or delay in performance of, any of our obligations under these Terms that is caused by an event outside our reasonable control.
9.13 If an event outside our reasonable control takes place that affects the performance of our obligations under these Terms, we will contact you as soon as reasonably possible to notify you, and our obligations under these Terms will be suspended and the time for performance of our obligations will be extended for the duration of the event outside our reasonable control.
9.14 You may cancel the contract if an event outside our reasonable control takes place and you no longer wish us to provide the Services. Where you cancel any of the Services due to an event outside of our control and you are a Private Patient, no cancellation fee (as set out at Section 9.7) will be applied.
The Contract
9.15 The following documents together form the contract for the Services between you and us:
(a) these Terms;
(b) the Registration Form;
(c) the Outpatient Communication and/or Admission Letter (as applicable); and
(d) Circle Health Group Digital Platforms Terms and Conditions (where applicable and including documents referenced therein).
9.16 The documents listed in Section 9.15 make up the entire agreement between us and you for the Services that we provide as part of the relevant treatment pathway.
9.17 If there is any conflict or inconsistencies between the documents listed in Section 9.15 above, the following order of priority shall apply:
(a) these Terms;
(b) the Registration Form;
(c) the Outpatient Communication and/or Admission Letter (as applicable); and
(d) Circle Health Group Digital Platforms Terms and Conditions (where applicable and including documents referenced therein).
9.18 In some cases, the Hospital is operated by one of our Affiliates, and where this is the case, Circle Health Group Limited is acting as a disclosed agent to that Affiliate, and that Affiliate is acting as principal.
9.19 This contract is between you and Circle Health Group Limited or an Affiliate. No other person shall have any rights to enforce any of its terms. As mentioned in Section 2 above, you will have a separate contract with the Consultant(s) providing your care and treatment.
9.20 These Terms are governed by the law of England and Wales. You and we both agree to submit to the non-exclusive jurisdiction of the English courts, however, if you are a resident of Northern Ireland you may also bring proceedings in Northern Ireland, and if you are a resident of Scotland, you may also bring proceedings in Scotland.
9.21 We may transfer our rights and obligations under these Terms to another organisation, and we will always notify you in writing if this happens, but this will not affect your rights or our obligations under these Terms.
Changes to these Terms
9.22 We may change these Terms at any time, however, any changes to the Terms will only apply to any new Services that you may receive and will not apply to Services provided in connection with any care or treatment that you may be part through or currently receiving when the change to these Terms is made.
Your wish to end any care or treatment before it is completed
9.23 If you wish to end any care or treatment before it is completed, you may do so and if you are a self-pay patient, your rights to do so are set out in Section 5.3.
Information and how to contact us
9.24 It is important that you keep us updated of any changes in your contact details.
9.25 We are a company registered in England and Wales. Our company registration number is 02164270 and our registered office is 1st Floor, 30 Cannon Street, London, EC4M 6XH. Where we are acting for one of our Affiliates, we are acting as a disclosed agent and the principal is the relevant Affiliate.
9.26 The entity responsible for your care (either Circle Health Group Limited or an Affiliate) will be as specified in your Outpatient Communication and/or Admission Letter. If you have any questions or if you have any complaints, or you wish to contact us (for example, to cancel the contract), you can contact us at: [email protected]. You may also contact the Hospital where you are receiving care or treatment or us at the address set out in Section 9.25.
Payment options for self-pay patients
9.27 If you are a self-pay patient, we will ask you to make a payment in full and in cleared funds in advance of your admission to the Hospital. We may refuse admission if you have not paid in full.
Your rights to a refund on cancellation are set out in Sections 5.3 and 5.4. You can pay online or over the phone.
10. Definitions
10.1 When we use the words “we”, “our” or “us” in these Terms we mean a member of the Circle Health Group.
10.2 When we use the words “writing” or “written” in these Terms, this will include e-mail unless we say otherwise.
10.3 When the following words with capital letters are used in these Terms, this is what they mean:
Admission Letter: if you will be admitted to the Hospital (or other Circle Health Group location), the letter that we send to you detailing the care or treatment and where you are a self-pay patient, either the Fixed Price to be provided to you under the Fixed Price Package or an estimate for costs where the treatment is not included within a Fixed Price Package.
Affiliate: any hospital owned or operated by any member of the Circle Health Group (excluding Circle Health Group Limited) or a third party that is managed by Circle Health Group Limited under a management agreement.
Circle Health Group: means Circle Health Group Limited or any company under the same control or otherwise within the same group as Circle Health Group Limited.
Consultant(s): means the independent medical practitioners involved in your care and treatment, who may be physicians, surgeons, radiologists, general practitioners or anaesthetists.
Fixed Price: the price that you pay for the Fixed Price Package and which we set out where relevant in your Admission Letter and/or your Outpatient Communication.
Fixed Price Package: the care, treatment or procedure to be carried out at the Hospital, to which a Fixed Price applies as detailed in your Admission Letter or your Outpatient Communication.
Hospital: means any hospital operated by Circle Health Group Limited or any of our Affiliates.
NHS: the National Health Service.
Outpatient Communication: the verbal or written communication that we provide to you detailing the outpatient care or treatment and where you are a self-pay patient, either the Fixed Price to be provided to you under the Fixed Price Package or an estimate for costs where the outpatient care or treatment is not included within a Fixed Price Package.
Registration Form: the registration form signed by you prior to an outpatient appointment at or admission to the Hospital.
Services: the services that we and/or your Consultant(s) are providing to you as set out in your Outpatient Communication and/or Admission Letter.
Terms: these terms and conditions set out in this document.
