About Dr Malcolm Metcalfe

Personal Profile

Dr Malcolm Metcalfe began his cardiology career as a Research Fellow in Aberdeen before completing his specialist training in Glasgow, where he held Registrar and Senior Registrar roles. During this time, he worked alongside internationally respected experts, gaining broad experience in heart failure, coronary intervention, cardiac imaging, hypertension, and arrhythmias. His training took place primarily at the Western Infirmary, with additional time at the Royal Infirmary and Stobhill General Hospital.

A passionate researcher, Dr. Metcalfe has contributed to over 40 peer-reviewed publications and presented more than 100 research papers at conferences worldwide.

He has a long-standing interest in cardiac imaging, having served as Treasurer, Secretary, and Chair of the British Nuclear Cardiology Society, as well as Treasurer of its European counterpart. In Glasgow, he helped establish advanced tomographic imaging services and was a lead operator of early colour flow echocardiography systems, including transoesophageal and intracardiac imaging (ICE).

Upon becoming a Consultant Cardiologist in Aberdeen, Dr. Metcalfe introduced several innovations in coronary intervention and helped expand the electrophysiology service. He was also the first in Scotland to implant a specialist heart failure pacemaker.

His interest in clinical leadership grew from a belief that clinicians should guide service development. He progressed from Lead Clinician to Deputy Medical Director at NHS Grampian. In these roles, he oversaw service improvements, and supported academic appointments following a British Cardiac Society review.

As Chair of the Managed Clinical Network, he partnered with Milne Weir (now Head of the Scottish Ambulance Service) to launch pioneering community cardiology services—many of which continue today.

Later, as Clinical Lead for the North of Scotland, Dr Metcalfe played a key role in establishing Inverness as a coronary intervention centre, significantly improving access to care for the region.

He returned to the acute sector as Medical Director and now serves as Deputy Medical Director of NHS Grampian. His responsibilities include acting as Deputy Caldicott Guardian, Chairing the Clinical Risk Meeting, and leading a group focused on improving outcomes for women with ovarian cancer.
