Private GP Membership Scheme – terms and conditions (“Terms”)

When we use the words “we”, “our” or “us” in these Terms we mean Circle Health Group Limited, a limited company incorporated and registered in England and Wales with company number 02164270 whose registered office is at 1st Floor, 30 Cannon Street, London, EC4M 6XHYou can find everything you need to know about us, and our services on our website https://www.circlehealthgroup.co.uk/

When you subscribe to the Private GP Membership Scheme at Syon Clinic, you agree to the following terms and conditions:

1. The Scheme Agreement

1.1. This agreement is between you and us.

1.2. The Private GP Membership Scheme at Syon Clinic is an annual subscription plan priced at £572 per individual.

1.3. The Private GP Membership Scheme is only available at the following Circle Health Group hospital:

Syon Clinic, 941 Great West Road, Brentford, TW8 9DU

1.4. Membership provides access to a range of benefits, as outlined in section 5.

2. Membership Eligibility

2.1. Membership is available to individuals aged 0 and above.

2.2. Membership is non-transferable and applies only to the registered individual.

3. Membership Terms

3.1. Membership is valid for 12 months from the date of activation.

3.2. Your subscription will be automatically renewed at the end of the year, unless it is written that you do not want automatic renewal, or you have cancelled your appointment.

3.3. Renewal reminders will be sent 4 weeks before the expiry date.

3.4. You may cancel your membership for any reason by writing to us within 7 days of receiving the membership documents. You are entitled to a full refund if you have not used any of the benefits outlined in section 5.

3.5. You can end your membership at any time after the initial cancellation period of 7 days. If you want to end your membership, you must write to us providing one month’s notice. If you give us written notice on the first day of the month, your membership will end at the end of that same month. If you give written notice after the first day of the month, your termination date will be at the end of the following month.

3.6. If you cancel your membership after 6 months, no refund will be given.

3.7. If you cancel your membership within the first 6 months from the date of activation, you will receive a pro-rota refund. Refunds are calculated based on the full calendar months remaining between your cancellation date and six months after your membership year started. You’ll receive 1/12th of your annual subscription fee for each of these remaining months. If the value of the services provided to date exceed 1/12th of your annual subscription for each of the completed month, your refund will be adjusted by the accordingly. You will not be eligible for a refund if there are less than six months left in your membership year at the time of cancellation

4. Payment Terms

4.1. The annual membership fee of £572 is payable in full at the time of registration.

4.2. You may cancel your membership for any reason by writing to us within 7 days of receiving the membership documents. You are entitled to a full refund if you have not used any of the benefits outlined in section 5.

5. Membership Benefits

5.1. You are entitled to 5 GP appointments within a 12-month period from the date of activation.

5.2. You are entitled to receive 10% off GP appointments after your 5th appointment.

5.3. Outside of GP surgery hours you are entitled to access the Walk-in Centre when the GP member is over 18.

5.4. You are entitled to receive 10% off an annual Health Assessment package at the Syon Clinic.

5.5. You are entitled to an annual free flu vaccination.

5.6. You are entitled to free repeat prescriptions; subject to clinical need.

5.7. You are entitled to receive a 10% self-pay discount on all Syon Clinic services.

6. We may revise these Terms from time to time.

7. Personal information

7.1. We will need certain information from you that is necessary for us to provide appointments, and this can be asked for by anyone involved in your care or treatment, including by staff at the relevant Circle hospital, by the physiotherapist or other medical professional. If you do not, after being asked by us, provide us with this information, or you provide us with incomplete or incorrect information, we may not be able to provide you with the consultation.

7.2. Please note that your personal information will be used to process your membership to the Private GP Scheme. We are committed to protecting your personal data which we will use to provide the services you need. 8. Events outside of our control

8.1. We will make every effort to ensure that you can access all the entitlements of the Private GP Scheme Membership. However, we cannot promise this, and we reserve the right to cancel or change the date of your appointments.  There may be delays or cancellations for any reason, such as because of an event outside our reasonable control.  Where this happens, we will try to give as much notice to you as possible and our obligations under these Terms will be suspended and the time for performance of our obligations will be extended for the duration of the event outside our reasonable control.

8.2. We will not be liable or responsible for any failure to perform, or delay in performance of, any of our obligations under these Terms that is caused by an event outside our reasonable control.

9. General

9.1. We strongly advise that you avoid taking any valuables or cash to the relevant hospital when you attend your appointments.  If you do nonetheless bring any valuables or cash, this is at your risk as we do not accept any responsibility for the theft, loss of, or damage to, any of your or your visitors’ cash, valuables or any other property that you or your visitors bring to the hospital.

9.2. If there is any problem with the services we provide, please contact us and tell us as soon as reasonably possible, and we will investigate the problem under our complaints procedure and try to repair or fix the problem as soon as we can. Please ask any member of staff at the relevant hospital for a leaflet about our complaint’s procedure.

9.3. You have legal rights in relation to services not carried out with reasonable skill and care, or if the materials we use are faulty or not as described. Nothing in these Terms will affect these legal rights.

9.4. These Terms are governed by English law. You and we both agree to submit to the non-exclusive jurisdiction of the English courts.

9.5. If there is any conflict or inconsistency between any marketing materials and these Terms, these Terms will take precedence.

9.6. We may transfer our rights and obligations under these Terms to another organisation, and we will always notify you in writing if this happens, but this will not affect your rights or our obligations under these Terms.
