Your first appointment with an orthopaedic specialist is an important step in your journey toward getting expert help for your joint pain problems. During this consultation, your orthopaedic consultant will take the time to understand your symptoms, medical history, and specific healthcare needs.

The consultation will typically involve an in-depth discussion of how your symptoms impact your everyday life, along with a physical examination to assess the extent of your joint movement and function. Your consultant may recommend further diagnostic tests, such as an X-ray, MRI, or blood work, to confirm the nature of your joint issue and how it has affected your overall health.

If scans or tests are needed, you may need to return for a follow-up appointment to discuss the results. After a diagnosis has been made, your orthopaedic consultant will explain their findings in detail and outline the recommended treatment options available to you. This includes non-surgical options such as tailored physiotherapy and steroid injection treatment, or surgical options like joint replacement and repair surgery. Your consultant will also discuss the cost of treatment, often offered as a fixed-price package.

Following your consultation, you will receive a letter to your home address summarising your diagnosis and treatment options, giving you the clarity and confidence you need to make your next steps.

At the Circle Cheshire Clinic, our brilliant team of orthopaedic surgeons can perform many treatment options for joint pain. This includes:

The excision of a ganglion

The excision of a ganglion cyst is a minor surgical procedure to remove a fluid-filled lump that typically forms near your joints, most commonly on your wrist or hand. During the procedure, your consultant will make a small incision over the cyst. They will carefully separate it from the surrounding tissues, ensuring the entire cyst and its stalk (which connects it to your joint or tendon sheath) are removed to minimise the risk of it growing back. After your consultant has removed the cyst, they will close the incision with stitches and apply a bandage or dressing over the wound.

Toe amputation

A toe amputation is a surgical procedure performed to remove one or more of your toes, usually due to severe infection, trauma, gangrene, or complications from conditions like diabetes or poor circulation. Your consultant will begin by making an incision around the base of your affected toe, carefully cutting through your skin, muscles, tendons, and, if necessary, the bone. Special care is taken to preserve as much healthy tissue as possible and to ensure proper blood flow to the surrounding area. Your consultant will remove any damaged or infected tissue surrounding your bone to prevent infection from spreading.

Endoscopic treatment for carpal tunnel syndrome

Endoscopic treatment for carpal tunnel syndrome is a minimally invasive surgical procedure used to relieve pressure on the median nerve in your wrist, which happens when your carpal tunnel passageway (a passageway containing nerves, tendons, and ligaments) is compressed. Your consultant will insert an endoscope, which is a thin tube with a camera and light attached to the end of it, into the incision to guide their movement as they perform the procedure. They will use specialist instruments to cut the transverse carpal ligament, which is pressing on the median nerve. This release alleviates the symptoms of carpal tunnel syndrome, which include numbness, stiffness, and tingling.

Dupuytren's dermofasciectomy with graft

This is a surgical treatment for Dupuytren's contracture, a condition that causes your fingers to permanently bend toward your palm due to thickened tissue under your skin. The procedure involves removing both the abnormal fascia (the connective tissue responsible for the contracture) and the overlying skin. After this tissue is removed, your consultant will use a skin graft from another part of your body to cover the wound. This approach is often used when the disease recurs after a previous surgery or when the contracture is more severe.
By removing a larger portion of tissue, including your skin, the contracture is less likely to return.

An open release of the sheath of a tendon

An open release of the sheath of a tendon is a surgical procedure used to treat trigger finger, a condition that occurs when your finger becomes stuck in a bent position due to the constriction of your tendon sheath. Your tendon sheath is a protective covering that surrounds your tendons and helps control the movement of your fingers and thumbs. In trigger finger, this sheath becomes thickened or inflamed, causing your tendon to become stuck, making it difficult to straighten your finger smoothly.

During the procedure, your consultant will make a small incision at the base of your affected finger or thumb. They will then locate and cut the A1 pulley (which is the part of your tendon sheath responsible for restricting movement). Once the pulley is released, they will test your finger's movement to ensure the tendon can glide smoothly, before closing the incision with stitches.
