How to participate

1.1 In order to enter this promotion, please complete the online form and enter the information requested.

1.2 All entries must include your full name, email address and post code.

1.3 Entries that do not comply with these terms and conditions will be void and will not be entered into this promotion.

1.4 Entrants can enter this promotion in person by completing the online form and entering the information requested.

1.5 You may only submit one entry per person to this promotion.

Start date

This promotion will start at on Tuesday 16th June at 08:00am.

Closing date

The closing date for this promotion will be Wednesday 17th June at 17:00. Entries received by the promoter after this time will be void and will not be entered into this promotion.

Selection of the winners

3.1 Winners will be selected at random from all valid entries for this promotion.

3.2 Winners of the prize draw will be the first entrant who randomly selects the prize from the lucky dip providing they have correctly submitted their information in accordance with these terms and conditions.

• A range of further function tests

If any tests are not clinically appropriate for you, your doctor will advise you.

Please note that transportation to and from Circle Cheshire Clinic is not included.

3.5 There is no alternative prize.

3.6 The prizes are non-transferable and do not include any additional costs and expenses.

4 Restrictions and requirements

4.1 Please note the following restrictions and requirements apply: Entrants must be over the age of 18 and a UK resident registered with a UK General Practitioner. The essential health assessment tests to be carried out will be subject to assessment by The Circle Cheshire Clinic clinical team for suitability.

4.2 Employees of the promoter and their immediate families are ineligible to enter this promotion.

5 Notification of the results and receipt of the prize

5.1 The promoter will notify the winners on 16th or 17th June 2026. If a selected winner does not respond to the notification within 28 days, the prize will be forfeited and the promoter will select an alternative winner from the remaining eligible entries in accordance with the process described above (and that winner will also have 28 days to respond to their notification).

5.2 The winner will receive their prize on or before 30th November 2026.

6 Publicity and personal information

6.1 Please note that your personal information will be used to process your entry, to contact you about your entry, and for the purpose outlined in paragraph 8.2 below. Please see our privacy policy click here for more details regarding how the promoter will use your personal information.

6.2 The surname and county of the winner can be obtained by sending a stamped, self-addressed envelope to the Business Development Team, Circle Cheshire Clinic, The Courtyard, Hall Lane, Wincham, Cheshire CW9 6DG.

6.3 If you do not want your surname and county included on the list of winners referred to above, please email [email protected] prior to the closing date of the promotion.

6.4 You may be asked to participate in publicity relating to this promotion if you are selected as a winner.

7 General

7.1 This promotion is operated by Circle Cheshire Clinic, The Courtyard, Hall Lane, Wincham, Cheshire, CW9 6DG.

7.2 Entrants hereby grant the promoter a non-exclusive, perpetual, worldwide licence to publish the winner (first name, first letter of surname, on Circle Cheshire Clinic Facebook, Instagram and LinkedIn accounts, and entrants waive all moral rights in their submissions for the benefit of the promoter.

7.3 While nothing in these terms and conditions will limit or exclude the promoter’s liability for death or personal injury caused by its negligence or for fraud, the promoter will not be legally responsible to entrants or winners for any losses that were not foreseeable to the promoter or to the entrant at the time of entry to the promotion or which are caused by a third party.

7.4 The laws of England and Wales apply to these terms and conditions, although if you are resident elsewhere you will retain the benefit of any mandatory protections given to you by the laws of that country.

7.5 Any disputes will be subject to the non-exclusive jurisdiction of the courts of England and Wales. This means that you can choose whether to bring a claim in the courts of England and Wales or in the courts of another part of the UK in which you live.
