Case managers at Circle Rehabilitation

Circle Rehabilitation works in partnership with the client (the patient), family, and all professionals, including case managers, to develop an individual rehabilitation programme. We prioritise an open, honest relationship with all legal professionals involved and take a holistic approach to the care of each client, with an understanding and appreciation of their legal case. 

Our approach is centred on the client's needs and goals. We believe in a collaborative process where case managers are integral to the rehabilitation journey and included at every step along the way. Case managers are invited to attend regular review meetings and are welcome to visit their clients at any time. This collaborative effort ensures that all stakeholders are engaged and working towards the best possible outcomes for the client.

Our services include initial assessments, ongoing evaluations, and tailored rehabilitation plans. Costs for these services are available on request, ensuring transparency and clarity for all involved parties. 

    To streamline communication and ensure consistency, we assign dedicated points of contact for each case. These professionals are available to answer any questions, provide updates, and address any concerns that may arise during the rehabilitation process.   • Comprehensive assessment and reporting

    We offer a comprehensive pre-admission assessment conducted by our multidisciplinary team (MDT). This assessment ensures that all aspects of the patient's needs are evaluated.

    A detailed schedule is provided to allow each member of our MDT to meet and assess the patient.

    Following this assessment, we provide an outcome report detailing recommendations and goals, including the proposed length of stay for either inpatient or outpatient rehabilitation.
